Check_Request_Form – All requests for expense reimbursement that were incurred for a PTA sponsored event must be made using this form.
Committee Proposal Form – Each chairperson is responsible for submitting this to the PTA Executive Committee at a minimum of 3 weeks prior to date of event.
Event Evaluation Form – Each chairperson is responsible for submitting this to the PTA Executive Committee by the end of the quarter in which the event was held.
Photo/Name Release Form – The Wake County Public School System is striving to maintain a high level of security for your child(ren) regarding web site development. Many schools wish to use individual photos or group photos in their web sites as well as a student’s full name. These photos and names are used in many ways to highlight a child’s school life.
PTA Code of Ethics – PTA Executive Board members agree to abide by the PTA Code of Ethics by signing the form and the PTA secretary keeps on file.
Wake County Fundraising Form – Must be completed by the Committee Chair for each event that will be raising funds. This must be submitted with the proposal 3 weeks prior to the event.
